For Patients
Referrals
When possible I prefer patients be referred to me so their primary care provider knows what is happening.
However in the light of the current medical climate, with so many not having access to a family doctor, I will see patients without a referral
In order that the clinic run efficiently:
-
Patients are seen in order of booking times and not in order of arrival.
-
24 hours notice of cancellation should be given. Failure to do this may result in a cancellation fee.
-
Patients who arrive later than their appointment time may not be seen that day, but rescheduled for another appointment.
Payment
OHIP covers the cost of the initial assessment. They also continue to pay for treatment of any varicose veins larger than 5 millimeters in diameter and causing symptoms. If there have been any changes made to your OHIP card, address or phone number please remember to inform the clinic staff.. The OHIP number does not change with a new card, but the Version Code (one or two alphabetical letters at the end) alters with each change.
Treatment not paid for by OHIP is payable by cheque, money order, or cash on a per session basis. Receipts are issued for insurance and tax purposes
Problems During Treatment
If you have acute problems or concerns during treatment, please feel free to call the office.
This includes evenings, weekends, and nights. The phone line is always open for ACUTE PROBLEMS related to your veins. This includes evenings weekends and nights. By using this service you save yourself a long wait in Emergency, or getting advice from someone not involved in the treatment.
Otherwise, for all bookings, business matters & general information there is someone in the office every weekday morning between 9.30 and noon. Non urgent matters are only dealt with during those times
Simply Dial 807-577-4888
That is the only number you need to know
